We understand that this can be a very distressing time and so have developed a holistic approach to end of life care.
Our extensive experience in palliative care services has enables us to develop an empathetic service which addresses the particular demands of providing care for individuals with a life threatening illness.
Each situation is unique with its own challenges but by combining best practice with extensive experience we are able to offer support every step of the way.
We ensure that we are up to date with best practice and are members of the National Council for Palliative Care. Our end of life services has taken onboard the independent review of the Liverpool Pathway, led by Baroness Neuberger, and all care and support is now based on an individual care plan that incorporates the following 5 priorities:
When it is thought that a person may die within the next few days or hours:
- This possibility is recognised and communicated clearly and decisions are made, and actions taken, in accordance with the person’s needs and wishes.
- Sensitive communication takes place between staff and the dying person, and those identified as important to them.
- The dying person, and those identified as important to them, are involved in decisions about treatment and care to the extent that the dying person wants.
- The needs of families and others identified as important to the dying person are actively explored, respected and met as far as possible.
An individual plan of care, which includes food and drink, symptom control and psychological, social and spiritual support, is agreed, coordinated and delivered with compassion.
Before undertaking roles in palliative care all our support and nursing staff receive specialist training in order to ensure our care is delivered professionally and with sensitivity.
